Intake Forms

Excited to get started very soon! Please keep the following in mind:

All new clients are required to fill out the NEW CLIENT health intake form . All questions are necessary. Please make sure to also have the policies/consent form filled out before appointment. If these are not filled out, you will be required to do so upon arrival. This will cut into service time, with no price adjustments. Returning clients just need to fill out the health update form and consent everytime. Please make sure to list any new products or health issues on the update form.
